Bhubaneswar: In a ground-breaking medical feat, the cardiology team at Manipal Hospital, Bhubaneswar, led by Consultant Cardiologist Dr. DibyaRanjanBehera,Consultant-Interventional Cardiologist has successfully performed Odisha’s first TAVI-in-TAVI (Transcatheter Aortic Valve Implantation) procedure. This revolutionary intervention was carried out on an elderly female patient who had previously undergone TAVI but developed severe aortic stenosis due to degeneration of the earlier valve.
The patient, who had been experiencing debilitating symptoms including shortness of breath and chest pain, was evaluated and found to have severe narrowing of her aortic valve. Aortic stenosis, a condition where the valve connecting the left ventricle to the aorta becomes obstructed, is a life-threatening disease that can lead to fainting spells, chest pain (angina), heart attacks, and respiratory distress if left untreated.
Traditionally, such conditions were treated through open-heart surgery to replace the damaged valve. However, for elderly patients with multiple comorbidities, open-heart surgery poses significant risks. Recent advancements in medical science now allow for Transcatheter Aortic Valve Implantation (TAVI/TAVR), a minimally invasive procedure that replaces the valve using a catheter, eliminating the need for open-heart surgery.
What made this case particularly unique and complex was the presence of a degenerated bioprosthetic TAVI valve implanted a few years ago. Open-heart surgery post-TAVI would have required a Bentall procedure, a highly risky option in this case. Therefore, the team decided on the innovative TAVI-in-TAVI approach, using a balloon-expandable valve (Myval, Meril). The procedure, conducted six months ago, was a resounding success, and the patient has since been doing good.
